Submit completed form, together with national passport, to OHR for certification, then to TSS/GO/BMS.

The form should be signed by the certifying officer of the OHR unit servicing the staff member’s business unit.

The national passport should be valid for at least 6 months; if a visa is also requested for eligible dependents, their national passports should also be submitted.
